1. Are you currently experiencing any of these symptoms? Yes / No

• Fever (feeling hot to the touch, a temperature of 37.8 degrees Celsius or higher)
• Cough that's new or worsening (continuous, more than usual)
• Barking cough, making a whistling noise when breathing(croup)
• Shortness of breath (out of breath, unable to breathe deeply)
• Sore throat (not related to seasonal allergies or other known causes or conditions)
• Runny nose (not related to seasonal allergies or other known causes or conditions)
• Stuffy or congested nose (not related to seasonal allergies or other known causes or conditions)
• Lost sense of taste or smell
• Headache that’s unusual or long lasting
• Digestive issues like nausea/vomiting, diarrhea, stomach pain (not related to other known causes or conditions)
• Muscle aches that are unusual or long lasting
• Extreme tiredness that is unusual
• For young children and infants: sluggishness or lack of appetite

2. In the last 14 days, have you been in close physical contact (living in the same home and less than 2 metres away) with someone who either? Yes / No

• Is currently sick with a new cough, fever, or difficulty breathing?
• Is waiting a test result for COVID-19?
• Has tested positive for COVID-19?
• Returned from outside of Canada in the last 2 weeks and has symptoms of COVID-19?

3. Have you travelled outside of Canada in the last 14 days? Yes / No

NOTE:

In order to protect other brothers and sisters when we come into our church for in-person services, we would highly recommend that those who plan to come in for in-person services would first receive COVID-19 vaccination. This will ensure a safe and healthy environment for everyone who comes into our church.
